1.In regard to the purpose of use of personal information, the Company will use personal information for the business content listed below, as well as within the scope required to achieve the specified purpose of use.

2.Disclosure and Correction of Information
The Company will accept the following requests from you: Disclosure of the personal information that is subject to disclosure notification of the purpose of use; correction of your personal information because of a factual error in the personal information subject to disclosure; suspension of use, or cessation of disclosure to third parties (hereinafter "Disclosure, etc.").

- Request form for the notification and disclosure of the purpose of use of personal information
- Request form for the correction of personal information
- Request form for the cessation of use of personal information
- Documents for personal identification (driver’s license, passport, etc.)
- If a person making a request is a statutory agent of the individual concerned, please provide documents which certifies his/her authority of statutory representation as well as the documents indicated in 1 to 4.
- If a person making a request is an agent of the individual concerned, please provide the letter of proxy prescribed by the Company and the personal seal registration certificate of the individual concerned as well as the documents indicated in 1 to 4.
* Please ensure any sensitive information, such as your legal domicile, listed on the submitted documents is blacked out prior to submission. If such information has not been blacked out when the documents are received by the Company, it will be blacked out by the Company and regarded as not to be acquired.
(3) Processing fee
Please note the Company will charge a 1,000 yen processing fee for each request made for the disclosure of personal information and the notification of purpose of use.
For requests made via mail, handling charge will be applied for the operations such as bank transfer.

The Company will carry out the joint use of personal information as follows:
